for up to a $3000 sign-on bonus. You will work at 2275 Deming Way.
This is a hybrid position. You will work in person a minimum of 3
days per week. /n Pay: /n /n - The pay range listed reflects both
titles and is based on a 1.0 FTE. /n - This position may be

remarkable /n Working as an independent healthcare provider, you'll
play an integral role in our care teams by providing high-quality,
cost-effective care to patients while working collaboratively
within a multidisciplinary health team. /n We are seeking a
Behavioral Health Therapist or Clinical Psychologist to: /n /n -
Evaluate patients presenting with a wide range of mental health
concerns and diagnoses. /n - Collaborate with patients on
individualized treatment plans with specific goals and outcomes. /n
- Coordinate patient care with primary care, other specialty
providers, and families via written and verbal communication. /n -
Provide psychotherapy, utilizing multiple modalities and methods
consistent with evidence-based practice, including specific and
focused treatment protocols, when appropriate. /n - Utilize
progress monitoring and outcome measurement tools to assess
benefits of psychotherapy and change the treatment plan, if

Education: /n /n - Minimum – Master's Degree in Social Work,
Counseling, Psychology, Marriage and Family Therapy, or other
behavioral health related field /n /n Work Experience: /n /n -
Minimum – 2 years of experience providing evidence-based treatment
for a variety of behavioral health diagnoses for children,
adolescents, or adults /n - Preferred – 5 years of experience
providing evidence-based treatment for a variety of behavioral
health diagnoses for children, adolescents, or adults /n /n
Licenses and Certifications: /n /n - Licensed Clinical Social
Worker Upon Hire Required or /n - Licensed Professional Counselor
Upon Hire Required or /n - Licensed Marriage and Family Therapist
Upon Hire Required and /n - Basic Life Support/CPR within 6 months
Required /n /n If Clinical Psychologist: /n Education: /n /n -
Minimum – Doctor of Philosophy (Ph.D.) or Psychology (Psy.D.) in
Counseling or Clinical Psychology or related field. /n /n Work
Experience: /n /n - Minimum – One (1) year of postdoctoral
supervised clinical experience Preferred /n /n Licenses and
Certifications: /n /n - Licensure or eligible for licensure as a
Psychologist in Wisconsin /n - Basic Life Support/CPR within 6
