SSM Health, a leading non-profit healthcare organization, is seeking a dedicated full-time Registered Nurse (RN) for our Home Health team. This role is crucial in delivering high-quality patient care to those in the community who require medical support in their homes. The ideal candidate will be compassionate, independent, and well-organized, with a strong commitment to healthcare excellence in alignment with our Catholic values and mission. Provide direct and comprehensive nursing care to patients in their homes. Coordinate with physicians and healthcare team members to assess, plan, implement, and evaluate patient care plans. Educate patients and their families on proper home health care strategies and procedures. Administer medications and maintain accurate patient records. Monitor patient recovery and outcomes, adjusting care plans as needed. Strong clinical skills in a home care setting Excellent communication and interpersonal skills Proficiency in electronic medical record (EMR) systems Ability to work independently with minimal supervision Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license in the state of practice CPR certification Valid driver's license and reliable transportation
