Supervisor Reports to: Pathways to Success Program Manager Purpose:
The Pathways to Success Supervisor provides administrative and
clinical supervision to Behavioral Health Care Coordination team as
part of the Health Choice Pathways to Success program. This
position supports Care Coordinators in their day-to-day processes
and job responsibilities, provides training to assigned staff
members and assists in care coordination services to children and
families with behavioral health needs. Pathways to Success is a
State of Illinois initiative to provide comprehensive care
coordination that will enhance access to critical behavioral,
medical and social services for children with complex behavioral
health challenges. The program utilizes the evidenced-informed Care
Coordination and Support model. This is a structured approach to
care coordination that adheres to required procedures for child and
family engagement, individual care planning, identifying and
utilizing strengths and natural supports while monitoring progress
and fidelity to the required processes. Care Coordinators assist
the families in navigating the complex systems of care until they
can navigate it on their own. Provide clinical and administrative
support, direction, and supervision to care coordination staff
Provide culturally appropriate, competent, and individualized
supervision in accordance with each supervisee’s age, gender and
gender identity, sexual orientation, race, ethnicity, religious
beliefs, and culture Assist with outcome measures and indicators
Maintain compliance with clinical documentation, timelines, and
agency policies and procedures Actively seek consultation whenever
necessary Maintain compliance with confidentiality policies and
procedures Ensure that care coordinators meet the needs of youth,
including through scheduling, provision of safe and intentional
community-based care coordination with collateral support
resources, and development of effective work/life balance
Participate in training opportunities Initiate, cultivate, and
maintain professional relationships with human services and
government agencies, health service vendors, and public and private
groups to enhance service delivery and C4s presence in the
community Work collaboratively with identified partners and
stakeholders to develop a network of resources, including other
community agencies and organizations such as DCFS, schools,
hospitals, and primary care providers Participate in Child & Family
Team Meetings Maintain certification in High Fidelity Wraparound
Care Coordination, IMCANS, and other state required assessment
tools and evidence-based practices Participate in the hiring of
care coordinators and performance monitoring Coordinate with other
program Supervisors, Coordinators, and Directors as needed
Participation in HFS Pathways to Success provider meetings Perform
other related tasks as needed Our ideal candidate: Education and
Experience: MA/MS in Social Work, Counseling, Marriage and Family
Therapy, or a related field is required Licensed Practitioner of
the Healing Arts (LPHA) required. Those whose licensure is in
process will be considered Clinical experience working with
individuals and families living with mental health needs and/or
substance use Familiarity with care coordination and case
management best practice philosophies Excellent interpersonal,
verbal, and written communication skills Personal vehicle, valid
Illinois driver’s license, and insurable driving record is required

Ability to comprehend complex ideas and complex diagnostic
histories of consumers Understanding of personal and professional
boundaries Ability to plan, develop and manage multiple projects

Computer literacy Problem-solving skills Bilingual Spanish/English
